I'm getting this error: AUDIT CHGMAN PACKAGE RC=12 & SYNCH4! Can some one please help.

Google is your friend. Although I know very little about CHANGEMAN, Googling AUDIT CHGMAN PACKAGE RC=12 & SYNCH4 returns 27 hits, several of which appear to provide an answer to what your problem is.

The setup and so the DDname might differ at different shops, however - once you've submitted the "audit Job" for the changeman package - in the SYSOUT of audit Job search for DD name SYSUT2, select this. And search for SYNC or 'Legend and Summary Report', you'll get a listing detailing your problem.
